Due to the particularity of the medical industry, to ensure medical safety, according to the requirements of national governments and medical institutions, when you make an appointment and receive medical services, medical institutions will require you to establish a health record, during which your valid documents (including but not limited to ID card, medical insurance card, passport) number, name, contact phone number, date of birth, gender, medical fee method, education level, occupation, work unit, address will be collected; or add corresponding information according to the requirements of relevant government departments and specific community medical institutions. This information is only used to complete the diagnosis and treatment/appointment of medical institutions, not for any other purpose. If you choose not to provide or do not agree to our collection and use of the above personal information, you will not be able to use the medical services we provide.
In the process of medical service appointment, you need to authorize our platform to collect your patient information, including but not limited to patient's name, ID number, date of birth, contact information, current residence, affiliated street, detailed address. Among them, the ID number is necessary information for medical institutions to confirm the patient; the date of birth is to exclude vaccine and physical examination institution restrictions (some vaccine vaccinations have strict age restrictions and some institutions will restrict people under 16 from conducting physical examinations); the contact information you provide is used to receive relevant service confirmation messages during the medical service appointment process. In addition, to provide health services such as report interpretation and post-examination management, you need to authorize our platform to obtain and view your physical examination report information without disclosing personal privacy. If you choose not to provide or do not agree to our collection and use of the above personal information, you may not be able to use the medical appointment service.
You can purchase corresponding products and/or medical appointment services online on our platform. In the process of using this function, you may need to make payment. During the payment process, you need to authorize WeChat Pay for payment settlement, WeChat Pay may collect your third-party payment account (Alipay account, WeChat Pay account and bank card information) for payment verification, as well as collect your shipping address information, including consignee, address, mobile phone number. If you choose not to provide the above personal information, you will not be able to use the relevant payment functions, but it will not affect your use of basic appointment services (non-online payment types), browsing and search functions.
We will assist the questionnaire publisher to collect the questionnaire content you (as the questionnaire publishing object) actively fill in (hereinafter referred to as "questionnaire data", which may include your personal information according to the questionnaire data you fill in at that time). Please note that you (as the questionnaire publishing object) should independently judge the risk of providing corresponding questionnaire data to the questionnaire publisher. You can make rehabilitation service appointments online on our platform. In the process of using this function, you need to authorize our platform to collect your patient information, including patient's name, mobile phone number, ID number, city. Among them, the ID number is necessary information for medical institutions to confirm the patient; the contact information you provide is used to receive relevant service confirmation messages during the medical service appointment process. In addition, to provide order verification, follow-up sports rehabilitation and other health services, you need to authorize our platform to provide the aforementioned information to cooperative platforms without disclosing personal privacy. If you choose not to provide or do not agree to our collection and use of the above personal information, you will not be able to use rehabilitation service appointment and verification services.
In the process of opening, using membership services and related rights, we will collect personal information directly related to membership services in accordance with the principles of legality, legitimacy and necessity. When opening membership services, you need to provide mobile phone number, WeChat nickname and name and other information for creating membership accounts and verifying identity. In the process of using this function, you may need to make payment. During the payment process, you need to authorize WeChat Pay for payment settlement, WeChat Pay may collect your third-party payment account (Alipay account, WeChat Pay account and bank card information) for payment verification. We will not transfer your personal information to any company, organization or individual except in the following cases:
(1) Transfer with explicit consent: After obtaining your explicit consent, we will transfer your personal information to other parties;
(2) In the case of merger, acquisition or bankruptcy liquidation, if personal information transfer is involved, we will require the new company or organization that holds your personal information to continue to be bound by this user privacy policy, otherwise we will require the company or organization to re-seek authorization consent from you.
